Insurance We accept most insurances. Please be sure to bring a hard copy of your insurance card to each visit. If you don’t see your insurance listed please contact our billing department for specific information about your insurance Payments at the time of Visit: Co-Payments are required at the time of the visit If your plan has an unmet deductible, we require payment from $75 to $100 at the time of the visit depending on the level of service Patients without insurance are welcomed, but payment at the time of the visit is required Billing Policies A billing fee will be added to any account that doesn’t pay at the time of service as stated above. We accept Visa and Mastercard, cash and personal checks. Any check that is returned for insufficient funds will be charged additional fees. Overdue accounts will be charged a monthly billing fee. We make every effort to keep to our schedule, and we ask that you arrive about 15 minutes early to the appointment time in case your insurance or personal information needs to updated. If you miss your scheduled appointment, or if you cancel with less than 24 hours’ notice, you will incur a charge (not covered by insurance) |
